Purpose: The person in this position will provide support to the Grace Hospital Liaisons.
Key Responsibilities:
· Track referrals working with the team and Intake to ensure that all information is given to Intake so the referral can be processed.
· Meet with families to secure consents, explain the hospice benefit and ensure expectations are set.
· Work with hospital Case Management and Social Worker's along with the onsite Grace team
· Order DME and equipment at the direction of the Grace RN
· Other duties as assigned within the persons scope of practice by the Grace Hospital Liaisons
· Ensure that Grace Healthcare Services' Best Practices are incorporated into all job activities, and responsibilities.
Qualifications:
· A college degree in the healthcare field is a plus.
· Must have healthcare experience.
· Excellent customer service and communication skills
· Ability to multitask.
· Organizational skills
